29 CFR 1926.502(i)(4): Employer did not ensure that all covers were color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: (a) Colorado Mechanical Systems Inc, at 1232 S Hover Street, Longmont, CO: On and preceding 8/31/15 the employer did not ensure that all covers were color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard. Employees sawed holes in the steel roof decking of Shops E at the Village at Twin Peaks shopping center for installation of the ducting for rooftop air handling units (RTU). Dimensions of paired holes for each RTU were fourteen inches by twenty inches and eighteen inches by nineteen inches. The sheets of four foot by eight foot OSB used as covers for each paired floor holes were not marked with "HOLE" or "COVER". This condition exposed employees to a fall hazard.
