29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:  a)  Shaker;  On or about July 8, 2015 and at times prior thereto, the employer exposed employees who are required to blow out the shaker to 14 feet fall hazards in that the ladderway opening was not offset or guarded by a swing gate.

29 CFR 1910.265(c)(18)(i): Construction, operation, and maintenance of conveyors was not in accordance with American National Standard B20.1-1957, which is incorporated by reference as specified in Part 1910.6. Section 602 Pulleys, Sprockets, Sheaves, Drums, Blocks, states that all of these when located in working area where operators (other than maintenance men) are present, shall be arranged to prevent the possibility of injury due to hands or parts of clothing being caught between the belt and pulley, chain and sprocket, or cable and sheave, drum or block:    a) Drive for the Bark Waste Conveyor (chain and sprocket);  On or about July 8, 2015 and at other times prior thereto, the employer exposed employees to amputation/caught-in hazards in that employees were allowed to walk and work within 4 feet of an unguarded chain and sprocket that was positioned 3 feet above the ground.    b) Drive for the Bark Waste Conveyor (shaft with keyway);  On or about July 8, 2015 and at other times prior thereto, the employer exposed employees to caught-in hazards in that employees were allowed to walk and work within 4 feet of an unguarded shaft with keyway that was positioned 3 feet above the ground.    c) Drive for the Bark Waste Conveyor (belt and pulley);  July 8, 2015 and at other times prior thereto, the employer exposed employees to amputation/caught-in hazards in that employees were allowed to walk and work within 4 feet of an unguarded rotating belt and pulley that was positioned 3 feet above the ground.    d) Head drive for the Bark Waste Conveyor (shaft with keyway);  July 8, 2015 and at other times prior thereto, the employer exposed employees to amputation/caught-in hazards in that employees were allowed to walk and work within 2 feet of an unguarded shaft with keyway that was positioned 4 feet above the ground.

29 CFR 1910.305(b)(1): Unused openings in boxes, cabinets, or fittings were not effectively closed:  a)  Electrical installations below the headrig;  On or about July 8, 2015 and at times prior thereto, the employer exposed employees to electrical hazards in that employees were allowed to walk and work around the disconnect (480 V) for the Top Strain of the Bandsaw with an open hole that was not effectively closed.  b)  Electrical installations below the headrig;  On or about July 8, 2015 and at times prior thereto, the employer exposed employees to electrical hazards in that employees were allowed to walk and work around the Starter (480 V) for the Top Strain of the Bandsaw with 2 open holes that were not effectively closed.  c)  Electrical installations below the headrig;  On or about July 8, 2015 and at times prior thereto, the employer exposed employees to electrical hazards in that employees were allowed to walk and work around an electrical panel (220 V) with 2 unused openings present.  d)  Debarker cab;  On or about July 8, 2015 and at times prior thereto, the employer exposed employee working in the cab of the debarker to electrical hazards in that employees were allowed to walk and work around an electrical panel with 1 unused opening present.
