(a) 800 Foxcroft Ave, Martinsburg WV: Employees were accessing the first and second buck of a fabricated walkthrough frame scaffold by means of the scaffold frame leg to conduct staging activities, as observed on or about 09/01/2015. ABATEMENT CERTIFICATION REQUIRED

(a) 800 Foxcroft Ave, Martinsburg WV: The employees were utilizing a fabricated walkthrough frame scaffold to conduct material staging work. The employees were not trained by a qualified person on scaffold and the hazards associated with the type of scaffold being used and on the procedures to minimize such hazards, as determined on or about 09/01/2015. ABATEMENT CERTIFICATION REQUIRED
