29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    (a)        All American Pet Proteins, LLC at 259 30th Street, Greeley CO 80631:  On or before September 2, 2016, the employer had not established a respiratory protection program for all employees who are required to wear a Scott full face respirator with multi-purpose cartridge model: 7422-SC1.  Three employees have been assigned to provide First Responder support during their respective shifts in the event of an emergency caused by an uncontrolled anhydrous ammonia gas release. This condition exposed these employees to respiratory hazards without the proper medical evaluation, fit-testing, and training regarding respiratory protection.     Abatement Note:  The employer shall include in the program the following provisions of this section, as applicable:      (1)        Procedures for selecting respirators for use in the workplace;      (2)        Medical evaluations of employees required to use respirators;      (3)        Fit testing procedures for tight-fitting respirators;      (4)        Procedures for proper use of respirators in routine and reasonably foreseeable emergency situations;      (5)        Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators;      (6)        Procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators;      (7)        Training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations;      (8)        Training of employees in the proper use of respirators, including putting on and removing them, any limitations on their use, and their maintenance; and      (9)        Procedures for regularly evaluating the effectiveness of the program.

29 CFR 1910.134(d)(3)(iii)(B)(2): The employer did not implement a change schedule for respirators not equipped with an End of Service Life Indicator that would ensure cartridges were changed before the end of their service life:    (a)        All American Pet Proteins, LLC at 259 30th Street, Greeley CO 80631:  On or before September 2, 2016, the employer had not implemented a change schedule for Scott multi-purpose cartridge model: 7422-SC1 that is based on objective information or data that will ensure that cartridges are changed before the end of their service life.  This condition potentially exposed employees to respiratory hazards.    Abatement Note:  The employer may provide an air-purifying respirator provided the respirator is equipped with end-of-service-life indicators or a change schedule has been developed. Otherwise, the employer must provide an atmosphere-supplying respirator.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:        (a)        All American Pet Proteins, LLC at 259 30th Street, Greeley CO 80631:  On or before September 2, 2016, the employer had not developed a written hazard communication program which at least described how the criteria specified for labels and other forms of warning, safety data sheets, and how employee training will be met. This program should also include a hazardous chemicals list and methods used to inform employees of the hazards of non-routine tasks, such as but not limited, to the following:      (1) Ammonia gas release; and     (2) Exposure to corrosive chemicals used for cleaning and sanitation, such as but not limited to: sodium hypochlorite, soda ash, CHEM-FOAM Chlorinated Caustic Equipment Cleaner, and Foaming Acid Cleaner.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:        (a)        All American Pet Proteins, LLC at 259 30th Street, Greeley CO 80631:  On or before September 2, 2016, the employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard is introduced into their work area such as but not limited to anhydrous ammonia, sodium hypochlorite, and soda ash.       Abatement Note:      (a)        Employees shall be informed of:      (1)        Any operation in their work area where hazardous chemicals are present; and,      (2)        The location and availability of the written hazard communication program, including the required list(s) of hazardous chemicals, and material safety data sheets required by this section.      (b)        Employee training shall include at least:      (1)        Methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area (such as monitoring conducted by the employer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released, etc.);      (2)        The physical and health hazards of the chemicals in the work area;      (3)        The measures employees can take to protect themselves from these hazards, including specific procedures the employer has implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment to be used;      (4)        The details of the hazardous communication program developed by the employer, including an explanation of the labeling system and the material safety data sheet, and how employees can obtain and use the appropriate hazard information.

This condition exposed workers to chemical hazards, cold stress, increased risk for skin infections, and slips or falls from improper foot wear.
