29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about September 2, 2016, an employee who performed servicing tasks was exposed to amputation hazards while performing inspection activities (troubleshooting) on the Form Fill Seal #3 and was not required to first perform the following sequence of actions: 1) Machine or equipment shutdown. 2) Machine or equipment isolation. 3) Lockout or tagout device application. 4) Verification of isolation. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
