29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: Located at 6199 Wilson Mills Rd, Highland Heights, OH: On or about September 14, 2015, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2015. 1. On or about September 5, 2015, an employee working in the garden center lost consciousness and was taken by emergency medical services to an area hospital and was admitted for dehydration. The Home Depot was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.4(a), which was contained in OSHA inspection number 843903, citation number 1, item number 1 and was affirmed as a final order on March 22, 2013, with respect to a workplace located at 92 Newbury Street, Danvers, MA 01923.
