The description recorded as "Sprain lower back" does not adequately describe "Employee was using a beam turner to roll a 941 lb beam and sprained lower back" as required in Column F on the OSHA  Form 300.  b)  On or about 09/15/2015, a welder, while pushing and pulling beams, sustained a chest wall pain (case log #15-07). The description recorded as "Chest wall pain" does not adequately describe "Pushing and pulling beams in place and felt chest wall sprain" as required in Column F on the OSHA Form 300.  c)  On or about 09/15/2015, a material handler, while walking, sustained a sprained right knee (case log #15-08). The description recorded as "Sprained right knee" does not adequately describe "Was walking and slipped on steel shot and sprained right knee" as required in Column F on the OSHA Form 300.  d)  On or about 09/15/2015, a welder, while rigging, sustained a crushed middle finger (case lon #15-10). The description recorded as "Crushed middle finger -Called OSHA on 07/07/15 at 8:17 am to verify if needed to be reported and Ron said no" does not adequately describe "Crushed middle finger while rigging a heavy girder with a J hook, didn't realize he was on a stiffener and the J hook turned and pinched his finger" as required in Column F on the OSHA Form 300.  e)  On or about 09/15/2015, a welder, while picking a pile, sustained a strained right knee (case log #15-11). The description recorded as "Strain right knee" does not adequately describe "Strain right knee while picking a pile of beams" as required in Column F on the OSHA Form 300.  f)  On or about 09/15/2015, an OTR driver, while chaining his load down, sustained heat exhaustion (case log #15-12). The description recorded as "Heat exhaustion" does not adequately describe "Heat exhaustion while chaining down load" as required in Column F on the OSHA Form 300.  g)  On or about 09/15/2015, a welder, while welding, sustained a strained left trapezius (case log #15-13). The description recorded as "Strained left trapezius" does not adequately describe "While welding twisted and strained left trapezius" as required in Column F on the OSHA Form 300.   h)  On or about 09/15/2015, a loader, while readjusting banding, sustained a laceration to the left middle finger (case log #15-14). The description recorded as "Laceration to left middle finger" does not adequately describe "Readjusting banding on load causing a laceration to the left middle finger" as required in Column F on the OSHA Form 300.
