29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a.)    Lewco, Inc. located at 706 Lane St. Sandusky, OH; On or about September 15, 2015, the employer did not assure employees utilized energy control procedures while changing blocks (punches) on the Bliss 7/16th press. Employees were required to change the blocks on the press and did not properly deenergize the press, exposing them to amputation hazards.           a.)    Lewco, Inc. located at 706 Lane St. Sandusky, OH; On or about September 15, 2015, the employer did not assure employees utilized energy control procedures while performing setup operations and cleaning the YCI-Supermax Vertical Mill. Employees were required to setup operations and clean the metal shavings in the mill and did not properly deenergize the mill, exposing them to caught by and amputation hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:     a.)    Lewco, Inc. located at 706 Lane St. Sandusky, OH; On or about September 15, 2015, the employer did not ensure employees were trained in the purpose and function of the energy control program and understood by employees operating the YCI-Supermax Vertical Mill.  Employees are required to setup and clean inside the mill and are exposed to struck by hazards from unexpected start up of the equipment.

29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics shall be provided to the operator when:   The operator has been involved in an accident or near-miss;     a.)    Lewco, Inc. located at 706 Lane St. Sandusky, OH; On or about September 15, 2015, the employer did not assure powered industrial vehicle operators who were involved in an accident or near miss were provided refresher training. In July, 2015 an employee punctured an oil reservoir on the staking press with the forks of the forklift, which resulted in spilled oil and a replacement reservoir. The employee was not retrained exposing employees to struck by hazards.
