Lewco, Inc. located at 706 Lane St. Sandusky, OH; On or about September 15, 2015, the employer did not assure employees utilized energy control procedures while performing setup operations and cleaning the YCI-Supermax Vertical Mill. Employees were required to setup operations and clean the metal shavings in the mill and did not properly deenergize the mill, exposing them to caught by and amputation hazards.
