29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. LEWCO, Inc. - Sandusky, Ohio: On or about January 23, 2017, employees that operated the Roper Whitney, Inc. roll bender machine (model 3418G; s/n 2511181) were exposed to amputation injury. The employer did not ensure the machine's point of operation was adequately guarded. b. LEWCO, Inc. - Sandusky, Ohio: On or about January 25, 2017, the employer did not ensure that the point of operation of the Roper Whitney, Inc. roll bender machine (model 3418G; s/n 2511181) was adequately guarded to protect against crushing and amputation hazards. An employee sustained a finger-tip amputation when operating the machine to bend metal.
