29 CFR 1910.212(a)(3)(ii): Point of operation of machinery was not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. The shear guard, on the V-die shear in building 1, was in disrepair and did not prevent an operator from placing a hand or body part in the point of operation. b. In building 2, the shear was not guarded. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
