29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition: a) 3rd floor of BOP shop, 'R' vessel hood, Braddock Avenue and 13th Street, Braddock, PA 15104: On or about September 16, 2015, the employer did not ensure that fall protection was provided, used and maintained by employees who were engaged in cleaning activities on the Jbends and header areas of the 'R' vessel hood. The employees were exposed to a fall hazard of approximately 58 feet to the pit below while cleaning the third floor of the BOP shop near the 'R' vessel hood and while accessing the header area from the third floor of the BOP shop.
