29 CFR 1910.146(d)(4)(iii): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not provide at no cost to the employees communications equipment necessary for compliance with 29 CFR 1910.146(h)(3) and 29 CFR 1910.146(i)(5): a) In the BOP Shop, on or about September 16, 2015 - The attendant was unable to maintain contact as necessary to monitor either entrant's status, nor where the entrants able to communicate with each other while they were inside the R Vessel hood cleaning and removing debris from the J bends and header because the employer did not provide the attendant and entrants with communications equipment. One entrant's whereabouts and status were unknown to the other entrant and the attendant for approximately fifteen minutes.
