Two of the eight outlets were burned.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    a) Indoor shop near workbench- On or about September 16, 2015, Employees were exposed to shock and burn hazards while using the HMD904 Magnetic Drill to drill holes into the fabricated steel. The power cord to the magnetic drill was damaged and the strain relief was pulled exposing the open conductor wires.     b) Indoor shop near workbench- On or about September 16, 2015, Employees were exposed to shock and burn hazards while using the Millermatic 252 MIG Welder to weld fabricated steel. The power cord to the welder was damaged and the strain relief was pulled from its' connection plug exposing the open 230 Volt conductor wires.
