29 CFR 1910.134(d)(1)(iii): The employer shall identify and evaluate the respiratory hazard(s) in the workplace; this evaluation shall include a reasonable estimate of employee exposures to respiratory hazard(s) and an identification of the contaminant's chemical state and physical form. Where the employer cannot identify or reasonably estimate the employee exposure, the employer shall consider the atmosphere to be IDLH.      Location - 18 Ellison Road, Lexington, MA:  On or about September 16, 2015, the employer did not evaluate respiratory hazards for employees who were using an abrasive  saw to cut stone veneer.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.    Location - 18 Ellison Road, Lexington, MA:  On or about September 16, 2015, employees were not protected from falling while installing stone veneer from a roof of a residential construction project where height was greater than 11 feet 7 inches to the ground.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.    Location - 18 Ellison Road, Lexington, MA:  On or about September 16, 2015, employees using a ladder that did not extend 3 feet beyond the roofs surface.
