29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury.  (a) Location - 7 Greenbriar Drive, North Reading, MA   On or about 12/21/21, Employee were not informed of the hazard of operating combustion powered equipment and the potential for carbon monoxide exposure and poisoning.  Employees were not instructed in the use of ventilation equipment to ensure carbon monoxide levels were reduced in the work space.

29 CFR  1926.55(b): Feasible administrative or engineering controls were not implemented to reduce employee exposure to a substance listed in Table 1 or 2 of �1926.55:  (a) Location - 7 Greenbriar Drive, North Reading, MA   On or about 12/21/21, employees were exposed to carbon monoxide in excess of the permissible exposure limits and the employer did not implement all feasible engineering and work practice controls, including administrative controls, to reduce employee's exposure to carbon monoxide.
