29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: a. Located at 7330 Groveport Road in Groveport, Ohio: On or about September 16, 2015, employees were removing exterior wall panels from the north wall of the recreation center from a Safeway Systems scaffold that was not fully planked, exposing employees to an approximately nine foot six inch foot fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as used as a means of access. a. Located at 7330 Groveport Road in Groveport, Ohio: On or about September 16, 2015, employees were using the scaffold frame to access the working platform of a Safeway Systems scaffold. The scaffold frame was not an approved access, thereby exposing employees to a maximum fall hazard of approximately nine foot six inches.
