29 CFR 1910.252(b)(3): Employees exposed to the hazards created by welding, cutting, or brazing operations were not protected by personal protective equipment in accordance with the requirements of 29 CFR 1910.132: (a) In the production area, on or about September 16, 2015 and at times prior to, the employer did not enforce the use of long sleeve shirts or protective sleeves for employees who engaged in welding activities, resulting in thermal and radiation burn hazards.
