29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  a) 1 Mill Valley Rd., Hadley, MA: On or about September 15, 2015 an employee was working from a Baker type mobile scaffold without any of the 4 wheels being locked.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) 1 Mill Valley Rd., Hadley, MA: On or about September 15, 2015 employees were exposed to falls in excess of 8 feet to the ground below while;    1) Placing/ plumbing an exterior wall   2) Building an architectural element within the structure.      Wilcox Builders, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926 501 (b) 1, which was contained in OSHA inspection number 315515080, citation number 1, item number 1 and was affirmed as a final order on February 13, 2012 , with respect to a workplace located at 7 Porter St., South Deerfield, MA.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) 1 Mill Valley Rd., Hadley, MA: On or about September 15, 2015 an employee was climbing on the top step of a 6 foot fiberglass A frame ladder to access the  Baker type mobile scaffold he was working from to accomplish his work.  Wilcox Builders, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926 1053 (b) 1 which was contained in OSHA inspection number  943645, citation number 1, item number 2 and was affirmed as a final order on January 22, 2014, with respect to a workplace located at 121 N. Pleasant St., Amherst, MA.
