29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level:  a) University Dr S and Rt 9 Hadley, MA: On or about November 9, 2021, two employees were observed working on the upper floor of a mixed use building being constructed with fall protection that was not rigged  such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record:  a) University Dr S and Rt 9 Hadley, MA: On or about November 9, 2021, the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained,  the signature of the person trained, date(s) the training was completed and the signature of the person who conducted the training for each employee exposed to fall hazards.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
