29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Spangler Candy Company, Bryan, Ohio; On or about September 18, 2015, the employer failed to ensure that employees operating the Hansella Werke Candy Cane Lines were protected from ingoing nip points created between Hansella sizing wheels and guide plates. Employees were exposed to injuries associated with caught in hazards such as but not limited to amputations.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Spangler Candy Company, Bryan, Ohio; On or about August 31, 2015 the employer failed to ensure that machine specific energy control procedures were developed, documented and utilized to control the electrical energy sources when employees remove jams on C7 Servomatic located in the Dum Dum department.  An employee sustained a fracture to their right hand when their hand was pulled in between a roller and the conveyor while inspecting the rollers post jam removal.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  a) Spangler Candy Company, Bryan, Ohio; On or about August 31, 2015 the employer failed to ensure that employees isolated energy sources on the R.G. Torino-Italia Ruffinatti C7 Servomatic located in the Dum Dum department prior to unjamming candy batches. Employees were exposed to caught-in and crush hazards if the rollers were to unexpectedly start while clearing a jam.
