Three employees installing underlayment paper and moving 2 x 6 planks on a 6:12 pitch roof are exposed to an 22 feet fall hazard without the use of personal fall arrest system, safety net system or guardrail system.      The employer fails to assure effective protection against falls as required by the standard, as demonstrated on September 21, 2015, when employees were working on a residential roof at Tamaya Lot 122, Jacksonville, Florida without fall protection.      Professional Sunshine Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or it's equivalent standard, 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1029787, citation number 01, item number 001 and was affirmed as a final order on April 22, 2015, with respect to a workplace located at 13007 Trave Way, Jacksonville, Florida.    Professional Sunshine Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or it's equivalent standard, 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1002905, citation number 01, item number 001 and was affirmed as a final order on November 20, 2014, with respect to a workplace located at 3632 Valverde Circle, Jacksonville, Florida.
