29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:    a) Third Floor Hallway - A carpenter was using an extension cord to supply power to a Craftsman Power Miter saw, model number 449946, that was plugged in to an outlet receptacle,15 amp, 120 volt service, missing the cover plate, on or about 09/22/2015.         No abatement certification or documentation required for this item.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) Parking Garage Roof - An employee was observed standing near the edge of a roof while installing foam onto the face of the building and exposed to an 11' 3" fall hazard, on or about 09/22/2015.    b) Exterior, N.E. Corner Walkway - Employees were standing on the walkway retaining wall while dumping gravel into the walkway and exposed to a 7' 5" fall hazard, on or about 09/22/2015.       In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:    a) Exterior, N.E. Corner Walkway -  Two pieces of unprotected rebar in the walkway where two employees were standing on the walkway retaining wall dumping gravel into the walkway and two employees were spreading the gravel as it was being dumped, exposing the employees to an impalement hazard, on or about 09/22/2015.         No abatement certification or documentation required for this item.
