29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded:  a) 111 S. State St., Ephrata, PA Construction Site - On or about September 23, 2015, the Dewalt gas-powered air compressor was not provided a guard for the drive belt and pulley exposing employees to entanglement injuries.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(1): Each employee on a working/walking surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet ( 1.8m ) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) 111 S. State St., Ephrata, PA Construction Site - Employee(s) were observed doing leading edge work from a low slope roof at a height of approximately 15 feet above the ground below without means of fall protection in place on or about September 23, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): 29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigged support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  a) 111 S. State St., Ephrata, PA Construction Site - Employee(s) were utilizing a wooden job made ladder to access the roof of the building. The ladder did not extend past the landing and was not provided with a grab rail on or about September 23, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.
