Service Wire Company, Warehouse:  The barrier guard and safety mat, in place, were not positioned in a manner to prevent an employee from making contact with a rotating axel on the SW-111 Rewinding machine, as determined on or about 09/24/15.    ABATEMENT CERTIFICATION IS REQUIRED.
