29 CFR  1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained.  a) SW 530; On or about April 7, 2021, and times prior, each authorized employee performing maintenance did not affix their own lock to a lockout device and was working under one employees  lock exposing them to amputations, head trauma, and death.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  a) SW 24 Strander; On or about May 13, 2021, and times prior, employees worked around and walked near the orbital bobbins.  The bobbins were not completely guarded, allowing employees to come in contact with them as they rotated, and be exposed to crushing and or amputation injuries.     b) SW 24 Strander; On or about May 13, 2021, and times prior, employees worked around a rotating strander that had a broken interlock on the guard.  Employees were exposed to rotating parts that cause  crushing and or amputation injuries.  c) SW 24 Strander; On or about May 13, 2021, and times prior, employees removed a guard to change gears and did not replace the guard prior to operating the machine, leaving the gears exposed.  Employees were exposed to rotating parts that cause  crushing and or amputation injuries.

29 CFR  1904.29(b)(1):  A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation.  a) 300 Davis Road; On or about May 7, 2021, the 2021 OSHA 300 log was not completed correctly in that the location of the event was not detailed enough to find the exact location or machine center involved and the injury or illness was not described in detail as to the specific part of the body that was directly injured and what it was injured by.
