29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: Total Grain Marketing, LLC had employees exposed to slip and fall hazards from an unguarded floor hole between Bin #6 and Bin #5. This floor hole could be guarded by either: a. A standard railing or b. A floor hole cover of standard strength and construction.

29 CFR 1910.27(d)(2)(i): Where a man had to step a distance greater than 12 inches from the centerline of the rung of a ladder to the nearest edge of structure or equipment, a landing platform was not provided. The minimum step-across distance shall be 2 1/2 inches: Total Grain Marketing, LLC had an employee exposed to fall hazards while sealing a bin door, and the landing platform did not have a minimum step across distance of 2.5 inches.
