29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  a. At the jobsite, the portable extension ladder used to access to roof was not secured from displacement, exposing employees to an 11 foot fall hazard, on or about 9/24/2015.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On the 5/12 pitched roof, fall protection was not provided to employees doing framing and installing plywood sheeting, exposing employees to an 11 foot fall hazard, on or about 9/24/2015.  The Fuerza Pilar Corporation Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1018537, citation number 1, item number 1 and was affirmed as a final order on 3/13/2015, with respect to a workplace located at 28625 Santa Luce Drive, Wesley Chapel, FL 33543.   The Fuerza Pilar Corporation Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1019541, citation number 1, item number 1 and was affirmed as a final order on 3/3/2015, with respect to a workplace located at 8840 Tropical Palm Drive, Tampa, FL 33626.   The Fuerza Pilar Corporation Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 991644, citation number 1, item number 2 and was affirmed as a final order on 2/24/2015, with respect to a workplace located at 7035 Peregrina  Loop, Wesley Chapel, FL 33543.   The Fuerza Pilar Corporation Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1013366, citation number 1, item number 1 and was affirmed as a final order on 1/27/2015, with respect to a workplace located at 11625 Brighton Knoll Loop, Riverview, FL 33579.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. At the jobsite, the portable extension ladder used to access the roof extended approximately 15 inches past the landing surface, exposing employees to an 11 foot fall hazard, on or about 9/24/2015.
