On or about November 11, 2015, on Building #3, the employer did not ensure that lockout/tagout procedures for the control of hazardous energy were being developed, documented and utilized while conducting maintenance and repair work on the 637512 AccurShear machine, 717516 AccuPress press brake machine, Niagara E-10 Press, VTC 300C Mazak Cutting, among other equipment with potential to store energy, exposing employees to electrical, caught-in and amputation hazards.
