29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.  On or about December 09, 2022, in the Doors and Panels (D4) area of the machine shop, the Chicago Dreis and Krump Press brake did not have a guard, exposing employees to hazards such as those created by point of operation.  b.  On or about December 09, 2022, in the Sheet Metal Press (I4) area of the machine shop, the Hercules 4 x12 Radial Drill Press did not have a guard, exposing employees to hazards such as those created by point of operation and rotating parts.

29 CFR 1910.215(b)(9):   The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a.    On or about December 09, 2022, in the Machine Shop area of the facility(B9),  the employer exposed employees to struck by hazards, in that employees were required to operate a Bench Grinder that did not have the tongue guard adjusted to one fourth inch from the grinding wheel.   b.  On or about December 09, 2022, in the Assembly Section (H3) area of the machine shop, the employer exposed employees to struck by hazards, in that employees were required to operate a Bench Grinder that did not have the tongue guard adjusted to one fourth inch from the grinding wheel.

29 CFR  1910.217(c)(2)(i):   Every point of operation guard shall meet the following design, construction, application, and adjustment requirements of this section:  a.    On or about December 09, 2022, in the Sheet Metal Press (I4) area of the machine shop,  the employer exposed employees to amputation hazards in that employees were required to operate a Bliss Press, Equipment numbered 130-8005 which had a point of operation guard that did not prevent the entry of hands or fingers into the point of operation and did not comply with the maximum permissible opening of Table O-10.

On or about December 09, 2022, in the Sheet Metal Press (I4) area of the machine shop,  the employer exposed employees to amputation hazards in that employees were required to operate a Bliss Press, Equipment numbered 130-8005 which had a foot pedal mechanism not guarded from unintended operation or falling or moving objects by accidental stepping onto the pedals.

On or about December 09, 2022, in the Sheet Metal Press (I4) area of the machine shop,  the employer exposed employees to crushing and amputation hazards in that employees were required to operate a Bliss Press, Equipment numbered 130-8005 without the employer having inspected, tested and documented the condition of the clutch/brake mechanism, anti-repeat feature on a weekly basis as required by the standard.
