On or about September 24, 2015, at the Eagle asphalt rock crusher, the drive pulleys for the number 1 shaker conveyor were unguarded, exposing employees to an amputation hazard from the ingoing nip points of the double belt and pulley. b. On or about September 24, 2015, at the Eagle asphalt rock crusher, the drive pulley for the course conveyor was unguarded, exposing employees to an amputation hazard from the ingoing nip point of the belt and pulley. c. On or about September 24, 2015, at the Eagle asphalt rock crusher, the drive pulleys for the middle conveyor were unguarded, exposing employees to an amputation hazard from the ingoing nip points of the double belt and pulley. d. On or about September 24, 2015 at the rotary dryer, on the north side, the electric drive motor pulleys were unguarded, exposing employees to an amputation hazard from the ingoing nip points of the double belt and pulley.
