29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  (a) roof above the entryway - an employee was exposed to a fall of approximately 9 feet 4 inches to the ground while installing shingles, in that, the employee was using personal fall arrest systems without properly adjusting and rigging their rope grabs to prevent contact with the lower level. Violation observed on or about 9/23/15.   (b) roof - an employee was exposed to a fall of approximately 20 feet to the ground while installing shingles, in that, the employee was using personal fall arrest systems without properly adjusting and rigging their rope grabs to prevent contact with the lower level. Violation observed on or about 9/23/15.   Millard Roofing, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(16)(iii), which was contained in OSHA inspection number 701480, citation number 1, item number 1 and was affirmed as a final order on 01/18/13, with respect to a workplace located at Lot 31 & Lot 35 Oak Pond Street, Hawkes Pointe, Ruskin FL 33570.
