29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). a. At the Brookside Villas jobsite, lot # 1, where shingles were being installed on a 6/12 pitch residential roof, fall protection was not used exposing employees to a 10 foot, 3 inch fall, on or about 1/27/2016. Millard Roofing, Inc. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1015325, citation number 1, item number 1 and was affirmed as a final order on 05/12/15, with respect to a workplace located at 27062 Evergreen Chase Dr. Westley Chapel, FL 33544 Millard Roofing, Inc. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 907210, citation number 1, item number 1 and was affirmed as a final order on 08/5/13, with respect to a workplace located at 13129 Royal Pines Ave., Riverview FL 33579.
