29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: In the production area of the facility the employer has failed to ensure that employee(s) affix a lock or tagout device to each energy isolation device when replacing the roll of plastic bags and Teflon on the midstream bagger machine. This procedure requires the use of tools to remove the cutting blade and takes from 15 to 20 minutes to accomplish.
