29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about September 25, 2015, employees performing routine work activities were required to work with chemicals such as, but not limited to, Mineral Spirits, Isopropyl Alcohol, Inks, and Graphic 408, and the employer did not ensure that each affected employee used appropriate eye protection against chemical splashes to eye and/or eye irritation in the following instances:  a) An employee was required to work with the above listed chemicals while wearing personal prescription eye glasses and the employer did not provided or require adequate eye protection.   b) Employees were required to work with the above listed chemicals such and the employer did not require the use of eye protection.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   Employees were exposed to hazards such as skin, face, and eye irritants, while working with chemicals such as, but not limited to, Mineral Spirits, Isopropyl Alcohol, and Graphic 408 and the employer failed to ensure a written hazard communication program had been implemented and maintained within the facility.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.  Employees were exposed to hazards such as, skin, face, and eye irritants, while working with chemicals such as, but not limited to, Mineral Spirits, Isopropyl Alcohol, and Graphic 408 and the employer failed to ensure training had been provided to employees.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
