29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Shop, production machines including die cutting equipment - employees were assigned to perform servicing tasks in the point of operation and associated danger zone. Hazardous energy control (lockout tagout) procedures were not developed and utilized in the workplace. Training and periodic inspections were also not conducted. Employees were exposed to hazards capable of causing hand(s) and finger(s) amputations from unexpected start-up.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    Throughout the shop, powered industrial truck- employees were operating the Komatsu electric forklift without undergoing adequate formal, practical training and evaluation in safe operation of the equipment, exposing them and other employees working in the area to struck-by and crushing hazards. 29 CFR 1910.178(m)(2): Employee(s) stood under or passed under the elevated portion of a powered industrial truck(s)    Shop, Comco printing presses, powered industrial vehicle: employees were operating electric forklift as a hoisting device for replacing overhead rolls. Employees were required to place a roll on adjusted forks, and then elevate and align it close enough to press's mandrel, so that the roll can be pushed manually by hands onto it. Employees accessing the area under elevated load on forks were exposed to crushed-under hazards.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    (a) Shop, Comco printing press no. 1, color stations: operators were running printing operations on the equipment with missing top-lift access guards at color stations during operating cycle. Employees had their finger(s) and/or hand(s) exposed to hazards capable of causing amputation.     (b) Shop, Comco printing press no. 1, color stations: operators were running printing operations on the equipment that had multiple safety interlock devices by-passed during operating cycle. The  key-type safety interlocks on movable guards had the keys permanently inserted into their receivers, rendering them ineffective as designed and constructed. Employees had their finger(s) and/or hand(s) exposed to hazards capable of causing amputation.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
