29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Fabrication and fleet service areas, saws: employees were assigned to servicing tasks (blade changes) in the point of operation and associated danger zone. Hazardous energy control (lockout tagout) procedures were not developed and utilized in the workplace. Training and periodic inspections were also not conducted. Employees were exposed to hazards capable of causing hand(s) and finger(s) amputations from unexpected start-up. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
