29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Fabrication and fleet service areas, saws: employees were assigned to servicing tasks (blade changes) in the point of operation and associated danger zone. Hazardous energy control (lockout tagout) procedures were not developed and utilized in the workplace. Training and periodic inspections were also not conducted. Employees were exposed to hazards capable of causing hand(s) and finger(s) amputations from unexpected start-up.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Fleet servicing shop, Roll-In bandsaw - employees cutting metal stock on the equipment were exposed to hazards capable of causing amputation where the guard was missing to prevent access to the unused portion of the running blade.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Fabrication shop: employees operating the Porter radial saw for cross-cutting and ripping wood stock were exposed to having their finger(s) and hand(s) amputated where the point-of-operation guards and/or safeguarding devices were not provided to prevent access to the danger zone.
