29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about 09/17/15, at the jobsite located at 702 Hesemann Ridge Court, Wildwood, MO  63021, employees were applying shingles using a Hitachi, Model NV 45AB2, 1 3/4" Coil Nailer, without donning the necessary eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 09/17/2015, at the jobsite at 702 Hesemann Ridge Court, Wildwood, MO  63021, employee(s) engaged in residential roofing activities were exposed to a fall distance of approximately 9 feet, 8 inches as they worked without any form of fall protection on an approximate 9:12 pitch roof.
