29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    In the Special Fittings Department, the 10-inch collar machine had rotating parts that were not guarded, exposing employees to the amputation hazards while the machine was operational.    M & M Manufacturing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 316045236, citation number 1, item number 1 and was affirmed as a final order on June 26, 2012, with a final abatement date of June 18, 2012, with respect to a workplace located at 4001 Mark IV Parkway, Fort Worth, Texas 76106.    M & M Manufacturing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1044326, citation number 2, item number 1 and was affirmed as a final order on May 22, 2015, with respect to a workplace located at 5555 Guhn Road, Houston, Texas.    M & M Manufacturing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1060668, citation number 1 item number 1 and was affirmed as a final order on June 30, 2015, with a final abatement date of June 19, 2015, with respect to a workplace located at 360 S. Shiloh Road, Garland, Texas 75042.
