29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition. The employer failed to ensure that the pendant control that controls the feeder to the Multipress II, located in the coil department had its functions properly identified exposing employees to unexpected struck by or caught-in hazards while operating and/or performing tasks such as unjamming a press die.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer failed to ensure that lockout/tagout procedures were utilized by employees when performing tasks that require the machines to be locked out/tagged out. Employees performing tasks such as unjamming a press die, were exposed to amputation hazards. The M&M Manufacturing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(I), which was contained in OSHA inspection number 1044326, citation number 1, item number 2 and was affirmed as a final order on 05/22/2015, with a final abatement date of 09/16/2015, with respect to a workplace located at 5555 Guhn Rd., Houston, TX 77040.
