29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Production, Band area, Acrotech rolling machines: on or about September 11, 2015, and on subsequent dates, operators were assigned to roll sheet metal parts on the unguarded equipment with missing point-of-operation guards. Employees had their finger(s) and hand(s) exposed to caught-in hazards likely to result in amputation. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder (handheld angle grinder) were not provided with safety guard having a maximum exposure angle of 180 degrees and located between the operator and wheel: Casting department - employees using multiple portable vertical/right angle handheld grinders had their hand(s) and finger(s) exposed to hazards capable of causing amputation, and other body parts exposed to lacerations and struck-by hazards where abrasive wheel cup guards were not provided to prevent contact. There is no abatement certification or documentation required for this item.
