29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Lettershop Department- The employer did not ensure that employees performing servicing and maintenance on inserting machines were required to: (1) prepare the machine or equipment for shut down, (2) turn off and shut down the machine or equipment, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source(s), (4) affix lockout or tagout devices to each energy isolating device, and (5) verify isolation and deenergization of the machine or equipment have been accomplished prior to starting service and/or maintenance work. Employees were exposed to amputation hazards associated with the unexpected start-up and energizing of the inserting machine. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Lettershop Department - The employer did not ensure that the CMC 150 Inserting machine #22 was equipped with permanent panel guards during operation to eliminate employee(s) exposure to rotating belts and pulleys. On September 19, 2015, a temporary employee operating Inserting machine #22 suffered an amputation injury to their finger when they reached into the unguarded mechanical adjustment section and the employee's finger was caught between the belt and pulley. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification.
