29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: a) Shrink Wrap Department- On or about May 10, 2022, the employer did not ensure that all guards on machines, such as the Shenklin Automatic Seal Shrink Wrapper, were affixed and/or secured. The point of operation guard for the heat-sealing area was not secured or interlocked to prevent employees from accessing the area during normal production. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) Inserter Department - On May 11, 2022, the employer did not ensure that all flexible cords were inspected prior to use in order to identify hazards and ensure that they were free of defects and damage that could expose employees to injury. The outer jacket on the orange extension cord, used to power the power strip used for the Tabber machine, was separating from the electrical plug. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
