29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section:  a) On or about September 28, 2015, Nation Pizza Products, L.P.  did not ensure the energy control procedure for the Capway oven loader clearly and specifically outlined the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy including the following items:  A) A specific statement of the intended use of the procedure, B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy, C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     a) On or about September 28, 2015, before servicing the Capway oven loader, Nation Pizza Products, L.P. did not ensure the following steps for de-energization were achieved:     1) Prepare the machine or equipment for shutdown,  2) Turn off and shut down power to the machinery,   3) Physically locate and operate energy-isolating devices in such a manner as to isolate the machine or equipment from the energy source(s),   4) Affix lockout/tagout devices to each energy isolating device, and  5) Verify isolation and de-energization of machinery have been accomplished prior to starting the maintenance and/or servicing work.     An employee sustained an amputation injury from the unexpected start-up and energizing of the Capway oven loader.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    a) Nation Pizza Products, L.P. did not ensure that maintenance employees were adequately trained as "Authorized" employees and/or knowledgeable in the recognition of servicing and/or maintenance activities that require machine or equipment shutdown and lockout to be performed to prevent injury from operating equipment.  On or about September 28, 2015, an employee suffered an amputation injury when attempting to adjust the timing of the Capway oven loader conveyor in Bakery 1.    No abatement certification or documentation is required for this item.
