29 CFR 1910.178(l)(1)(ii):  The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a power industrial truck:     (a) The employer did not provide employees with adequate forklift training prior to allowing employees to operate and examine the forklifts. This exposed employees to struck-by hazards.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening whwere they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:    (a) The space in front of electrical panels in the facility, for lighting fixtures, were not maintained clear for a minimum distance of 3 feet (0.9 meters). This exposed employees to electrical shock hazards.
