29 CFR 1910.253(e)(3)(ii)(C)(3): Flashback protection was not provided by an approved device that would prevent flame from passing into the fuel gas system: On or about October 7, 2015, the employer did not ensure flashback arrestors were installed on cutting torches, this condition exposed employees to fire hazards in the following instances: a. Employee number one was cutting railroad steel with a Victor torch #3163339 without a flashback arrestor installed. b. Employee number two was cutting railroad steel with a Uniweld torch #831, 8F42 without a flashback arrestor installed.

Employee number one was cutting railroad steel with a Victor torch #3163339 without a back flow preventer installed. b. Employee number two was cutting railroad steel with a Uniweld torch #831, 8F42 without a back flow preventer installed.
