29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to de-energize circuits and equipment on which work was to be performed: On or about November 16, 2016, the lock used by maintenance to lockout the control valve of the bailer was removed before work was completed and no tag was used indicate tagout of the control valve emphasizing de-energized circuits and equipment. Employees were exposed to electrical shock hazards.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: On or about November 16, 2016, there was an electrical raceway on the east side of the hotbed electrical room . There were some electrical panels attached to the top of the raceway. The raceway had some live wiring feeding into it and it had several openings on it which were not effectively closed. Employees were exposed to electrical shock.
