A) Chemical Storage Room - On or about October 5, 2015, approximately 9 circuit breakers were missing from the electrical panel.  Eight of these openings had been covered with pieces of duct tape, which did not afford protection substantially equivalent to the walls of the panel box.  The ninth was uncovered and open.
