29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: A) Worksite - On and prior to October 5, 2015, suitable facilities were not available to flush the eyes and body of employees working with corrosive chemicals, such as but not limited to: Univar Blend CIP Cleaner. The employer had multiple 16 oz bottles mounted on the wall, which were not able to supply a 1.5 L/min flow of eyewash solution for 15 minutes.

29 CFR 1904.7(b)(3): An injury or illness involving one or more days away from work, was not recorded on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column: A) Worksite - On or about October 5, 2015, the employer did not accurately record the eye injury of the pasteurization employee, which occurred on August 28, 2015 and involved days away from work. The OSHA 300 needed to have the box checked for "Days Away from Work", the number of days needed to be entered in Column K, and the box in Column M1 needed to be checked for "Injury."

The employee was hospitalized for two nights.
