29 CFR 1910.253(e)(1): Equipment was not installed and used only in the service for which it was approved and as recommended by the manufacturer:  Equipment was not installed and used only in the service for which it was approved and as recommended by the manufacturer, in the following instances:  a)  At the Site - The connected individual oxygen and acetylene hoses lines were secured with small adjustable radiator hose clamps. b)  At the Site - An oxygen hose line had a blister on the outer layer. c)  At the Site - The preheat valve knob could no longer prevent the fuel gases from escaping from the hoses in the "OFF" position.
