29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    On or about October 6, 2015 and at times prior thereto, employees of Miguel Rodriguez were exposed to struck by hazards because they were using nail guns  without eye protection to replace a roof on a single story residential home at or near 16 Emerald Drive, Maumelle, Arkansas. This condition exposed employees injuries of the eye.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).    On or about October 6, 2015 and at times prior thereto, employees of Miguel Rodriguez were replacing the roof of a 6/12 single story residential roof without adequate fall protection at or near 16 Emerald Drive, Maumelle, Arkansas. This condition exposed employees to the hazards of falls to the ground below.

On or about October 6, 2015 and at times prior thereto, at or near the jobsite located at the single story residential home at 16 Emerald Drive, Maumelle, Arkansas, an employee was exposed to fall hazards without having received any training in the recognition of those hazards.
